Transcripts may be requested by fax, mail or in person at Green Valley High School.  Forms are available in the Main Office or you can download it below.  To order by fax or mail:

  1. Print Transcript Request Form (pdf)
  2. Complete and sign form.
  3. Include a copy of your photo ID.
  4. Fax or mail request and ID to:

Forms submitted prior to 10:00am on Thursday will be available for pick up on Friday.  You must bring photo ID to pick up your transcript.

When requesting a transcript to be mailed, you must include the
complete address.
